It is vital that we have all student forms filled out as soon as possible. The district utilizes Final Forms, which minimizes the reparative processes and filling forms out by hand.

For students returning to the CTC for their senior year, parents and students may go straight to https://adams-oh.finalforms.com Parents can login with the account they have previously created. Students can login with their school Google account and proceed though the form procuress.

For incoming juniors from North Adams, Peebles, or West Union High Schools, your simply need to login to Final Forms, https://adams-oh.finalforms.com, login and update and sign all forms presented.
