The Adams County Health Department will be surveying Adams County students in grades 7 and 10 using the CDC's Youth Risk Behavior Survey with some added questions about COVID-19. The survey may be reviewed at the school office.
The online survey will be given to North Adams students in grades 7 and 10 on Thursday, April 29, 2021. Parents may choose to have their child opt out of taking the survey by completing the form below and returning the form to the school office by Wednesday, April 28, 2021.
